Subject: [PATCH] fix(instances): the Add disk dialog is as wide as the other
 card dialogs

The Add disk dialog kept Bootstrap's default 500px width when it got
its source cards, so their help and the name's hint broke over two
lines and left gaps between the fields. It now opens large, scrollable
and full screen on a phone, as the New storage pool and New network
dialogs that use the same cards.
